These terms and conditions set out the expectations for the Service User (parent/carer) and the Therapist (Janet Mogridge). Ensuring both parties are protected in the unlikely event of a disagreement.
Please do not hesitate to contact me if you have any queries about the following information. Some parts of the terms and conditions might not apply to you but please read the whole document and sign. You need to sign the form before assessment and therapy can commence.
I work primarily outdoors delivering therapy in nature at the Wellbeing Orchard. I can also support your child in their educational setting e.g. preschool, nursery or school and by visiting home.
I do not work weekends. My hours are flexible and are subject to change with the appropriate notice. Every child is entitled to free NHS Occupational Therapy therapy if needed. If the child is already receiving NHS Occupational Therapy, I can liaise with colleagues in the NHS to ensure therapy targets are complimentary.
The Therapist (Janet Mogridge) and Service User (parent/carer) will agree a day, time and location for the child’s first appointment. The location will be at the Wellbeing Orchard unless otherwise agreed. The cost will also be agreed (see below). Following this assessment we will discuss next steps depending on the outcome of the assessment. This might be further therapy, monitoring/reviews or no further input. All options will be discussed and further input will be agreed together.
Following the assessment, I am able to provide a summary report on request (this will be an additional charge), which will include advice and targets if relevant, within 14 days of the assessment. You will be given a copy of the report and with your consent, I will also copy in all relevant professionals and agencies involved with your child (e.g. teachers, NHS Therapists, paediatricians etc).
Location, dates, times and frequency of therapy sessions will be agreed between the Therapist and Service User before commencing. Nature based Occupational Therapy is located at the Wellbeing Orchard. The course of therapy sessions will be reviewed every 6 weeks and may be adjusted depending on child’s needs and progress. Any changes will be discussed and agreed beforehand.
I specialise in providing assessment and therapy for children aged 2.5 yrs to 16 yrs old. Children will have challenges participating in everyday life skills. They may have a diagnosis such as Autism or ADHD. Children may experience attention, sensory, self esteem and/or coordination difficulties.

The appropriate fee is agreed between Therapist and Parent/Carer before the assessment session and will depend on the clients need.

Travel will be charged at 50p per mile from PE9 3BX (as per google maps). A return journey will also be charged at this rate. Travel location will be agreed upon before assessment and therapy commences and I have the right to refuse travel to a location that is considered too far from clinic base.
A summary of intervention can be provided at the end of a therapy block. I am also able to write a report to contribute information where evidence of needs is being collected. Reports are charged at an agreed hourly rate.
I do not provide medico-legal reports but I will signpost you to Occupational Therapists that do provide such reports if you require one.
Payment needs to be made at the time of booking. Mileage will be charged after a visit according to the distance covered using Google maps from PE9 3BX. Invoices for travel must be paid within 7 working days of being issued. Details of payments will be clear on the invoice. Fees for reports and programmes are due on receipt. No further visits will be made until an outstanding balance is paid in full. I can arrange invoices for educational settings.
In the first instance, I will contact you to remind you that payment is overdue. If after 7 days of being contacted and still no payment, therapy will be suspended and I reserve the right to refer the matter to a solicitor and commence legal action.
If I need to cancel I will let you know as soon as possible and reschedule the appointment for a time/date convenient for all. If you need to cancel the appointment, please let me know as soon as possible. No charge will be incurred for cancellations with more than 24 hours’ notice. Late cancellations (after 8am on the morning of the appointment) – 50 % of the session fee will be charged. Non-attendance i.e if you have not informed me, will be invoiced at the full rate of the booked session.
Unless otherwise agreed, the cost of any resources provided to you are included in the session fee. At times or if requested, I will give you recommendations of useful resources for you to buy.
I am registered with the Information Commissioner’s Office (ICO) as a Data Controller.
All details, case notes, reports and correspondence are stored securely and treated confidentially according to the GDPR.
The HCPC requires that complete, relevant and accurate notes are made following all appointments and after liaison with others involved in your child’s care. All information recorded will be stored on an electronic filing system called WriteUpp. Please refer to their website for specific data handling information. Handwritten notes will be destroyed or scanned into WriteUpp if these form part of the therapy or assessment. I will never disclose information about you/your child to third parties, unless this is needed to safeguard the child. I will liaise with other professionals involved with your child’s care, only with your permission to do so.
You may apply in writing to access an electronic copy of your child’s notes or to request modifications of any inaccuracies. Requests will be dealt with within 30 days and will incur a fee of £10. See the Privacy Policy for further details.
I have a full up to date DBS. If I have any concerns regarding the safeguarding of your child/children or others, I have a legal obligation to share that information with relevant professionals in line with the Safeguarding Children’s Act 2004
It is best practice that I liaise with other relevant professionals involved in your child’s care. This can include: Paediatrician, GP, Health Visitor, NHS and Independent Speech and Language Therapist, Occupational Therapist, Physiotherapist, ABA Tutor, Teaching Staff, Area SENCO, Case Worker, Portage Home visitor, Family Support worker, Psychotherapist. I will only share and seek relevant information that supports my decision making in assessment, treatment approaches and recommendations.
Email and texting is not a 100% secure method of communication, therefore I will only use your child’s first name or initials when I email you. With your consent, email will be used for correspondence, to remind you of upcoming appointments and to send letters, reports and other documents. We will agree upon a password when sending documents via email. I use a secure encrypted email account :

If this is not possible and you wish to take a complaint further, please contact the Health and Care Professions Council (HCPC).
